| The sponsor or the sponsor's spouse must sign the registration forms. Please bring a current copy of your orders. If your DEROS date has changed, we will also need a copy of your amended orders (or any other official documentation to support the new DEROS date). We also require a copy of your child's shot records. | |

Please schedule a time with our Registrar to collect your child's records. This only applies to students who are currently enrolled at Liberty Intermediate School. The registrar will need at least a two-week notice and a copy of your new orders. Online Preregistration is available now to parents of new and returning students and will serve as a mechanism to reduce the time to register students. Once parents have keyed in the registration data, important forms can be generated, including Form 600, Form 600a, Immunization, Health History, Student Records Request, Special Education and Internet Permission.
